forwarded calls

my phone is forwarding calls to a number 1-916-952-9980 in the background without my knowledge .

i tried to removed without success

Thanks but I tried different ways and looks like is disabled,but when I dialed again

*#67# and *#62#  still is showing same number except on *#21#( disabled)

to disable I dialed #21# after checked again and still that number lingers
