SIM card is being used by someone else

I called and spoke to a representative about using a old SIM card because my device wouldn’t stay on and I wasn’t sure why.After giving the representative my SIM card number I wanted to charge it to he said that that SIM card was being used on another line. Kindly confirm what needs to be done as I explained to him this was not possible as the SIM card is in my possession.

This is known as a form of identity theft how can I get this fixed.
