nonworking unlock code

I have an unusual issue that many cricket personnel have tried the unlock code in the system for my phone but it never works. Please help me.. Is there a way to find out if my phone has a defect that is preventing it from accepting the unlock code?  Also because the unlock code does not work I need a PUK code before trying the unlock code again
